Looks like cyano, I tried everything before finally giving in to using Chemiclean. I don't like using chemicals to fix things but nothing was working, added flow, multiple blackouts, shortened light periods, less feeding, multiple water changes, I could not get whatever was out of synch back into synch. So I gave in and did a dose of Chemiclean and it has been gone since.
