Help with Acan

Hi all. I’ve had this coral for almost a year now, and recently (little before last week) it started showing some kind of hole on the side. At first I thought it was a new mouth, as it was eating happily (ReefRoids), so I let it be. But then I started looking for pictures of Acanthastrea propagating (as it was sold to me as Acanthastrea), and realised it’s too big. Maybe it’s an Acanthophyllia… Which means it could be some kind of necrosis instead. What do you think? Acanthastrea or Acanthophyllia? Propagation or injury?

Nitrate: 6-8ppm
Phosp: 0 (old test, new one coming in the mail…)
Ca: 420
Mg: 1350
Kh: 9.2

Has anything changed? I'd be hesitant to make any rapid decisions with something like that. It may also be an acan echinata...
