After 14 months - this crawled out - ID?

I keep looking at my tank a lot and have seen many critters, some very small, living in the tank.

I just saw this thing crawling on the bottom of my tank for the first time since I started my tank 14 months ago. It is about 20 mm long.

What is it? Where has it been for the last 14 months?
