Looks a lot like a carpet anemone. Does it have a mouth near the center?

Got it since 2011 and almost dominated my whole tank

Looks like a species of green Galaxia or some sort of Pavona coral

Do the polyps retract? If so, can you get a pic of them closed so we can get a better look at the skeleton.

I had a similar one in the past. It's a plating Hydnophora instead of the usual branching one.
